Recent amendments to the Privacy Act require health practitioners in private practice to obtain consent from their clients to collect, use and disclose clients personal information. No contact with other professionals will be made without prior discussion with you resulting in your verbal consent. In emergency situations it may be necessary for your practitioner to collect personal information from relatives or other sources where it is not possible to obtain your prior express consent. It is asked that, where possible, your request be in writing. A charge may be imposed for photocopying and time involved in processing your request. Where you dispute the accuracy of the information that has been recorded, you are entitled to correct that information. It is Centre policy that all steps will be taken to record all of your corrections and place them with your file but not to erase the original record. A copy of the Centres Privacy Policy is available upon request. In summary, any personal details collected about you will be done so in confidence. The information will be stored securely and available for your inspection on request. The information will not be disclosed to a third party without your permission.
